Compassionate Support at Home by UnityLift

Launched in November 2025, the Support at Home program replaced both the Home Care Packages Program and the Short-term Restorative Care Programme. It expands access to essential services, assistive products, equipment, and home modifications, empowering individuals to live safely and independently in their own homes for longer.


At UnityLift, we provide coordinated support at your place to help you live independently. We have partnered with Triology Care and Care Connect to assist you with everyday tasks and help you live confidently. We also provide complex and intensive care to add convenience to your life.

What to Expect from Support at Home?

If you opt for Support at Home (SAH), you can access a list of services that enable you to live comfortably in your home. These include:

Clinical Supports: Nursing care, occupational therapy, and podiatry are covered.
Personal Tasks: We assist with showering, medication administration and transport. We also provide respite care.
Everyday Living: Our friendly staff assist with day-to-day activities such as cleaning, gardening, shopping, or meal preparation.

The program also provides culturally respectful support, including assistance to participate in cultural practices and improved access to Aboriginal and Torres Strait Islander health practitioners and community health workers.

Continuous Care at Home

Once you are approved for continuing Support at Home services, you will be placed into one of eight funded care categories based on your individual support requirements. This funding also covers care coordination. NDIS Assistance may include daily activities like housework, meal preparation, and transportation. For those with more advanced needs, services such as nursing care or in-home physiotherapy may be provided. Higher support levels allow for increased care within your home, enabling you to remain independent for a longer time.

Pathways Under the SAH

Apart from ongoing care, Support at Home provides 3 short-term pathways. 

Restorative Care: The Restorative Care Pathway offers short-term, goal-focused support designed to help you restore independence and improve daily functioning. Through coordinated allied health and/or nursing services, this pathway reduces the likelihood of needing long-term, intensive support and helps you get back to doing the activities you value most.
Assistive Technology and Home Modifications (AT-HM) Scheme: The AT-HM scheme provides upfront financial support to improve safety and accessibility within your home. It allows you to access approved equipment, assistive products, and home modifications based on your assessed requirements.
End-of-Life: The End-of-Life Pathway is designed to help you stay comfortably at home during the final three months of life by providing enhanced care and support. You can be referred for a fast-tracked assessment under this pathway even if you are not currently receiving Support at Home services.

Services Included Under Support at Home

The following are the services covered under SAH.

Household Assistance: Our staff will assist you with everyday household tasks such as cleaning, grocery shopping, laundry, and basic chores to keep your home running smoothly.
Home Maintenance: Our supports will ensure your home and outdoor areas remain safe, functional, and comfortable.
Meal Services: Nutritious, safe, and enjoyable meals and drinks are provided to support healthy eating and proper hydration.
Social and Community Participation: We encourage and assist you to stay socially active and engaged with your community and culture, either face-to-face or through digital platforms.
Transport Support: Reliable assistance to help you travel to medical appointments, social outings, and daily activities.
Coordinating the Care: Our support staff helps with planning, organising, monitoring, and updating your ongoing care and support services.

How Does SAH Work – The Process

Once approved, you’ll receive a letter outlining your funded services. When your funding starts, you can choose a provider that suits you. Your provider will tailor services to your needs and budget, and these can be adjusted or reassessed anytime if your circumstances change.

Your contribution is based on the services you use and your financial situation. You may need to share income details with Services Australia. You only pay for non-clinical services you receive.
